Introduction

The CCC System (2.5) offers a new approach for documenting patient care in an electronic health record (E.H.R.) system. It uses a coded Standardized Framework consisting of 21 Care Components for classifying its two interrelated terminologies – CCC of Nursing Diagnoses and Outcomes and the CCC of Nursing Interventions and Actions

The 21 Care Components document, classify, and track care based on the six steps of the Nursing Process Standards of Care recommended by the ANA (1998): Assessment, Diagnosis, Outcome Identification, Planning, Implementation and Evaluation. The six steps of the nursing process also correlate to the six steps of the CCC System.
